REPORT A CONCERN

The form below is for recording concerns about a child, or adult at risk, that involve actual or suspected physical, sexual, emotional or financial abuse, bullying, neglect or discrimination.  

If someone is in immediate danger, call the police on 999.

For low-level concerns (perhaps about rudeness or less than respectful behaviour at any Game4Padel venue or event) please contact our Welfare Officer directly:

Please complete this form electronically (or print out, fill in, scan or photograph and email to Carol as soon as possible, within 48 hours of the concern. Leave blank any bits you are unable to answer. Do not hold back from reporting a concern because you don’t know all of the details.
